Khoo Teck Puat Hospital, Yishun Health
Bio

Dr Wong is a geriatrician by training. She has spent her last 20 years actively promoting functional longevity and ageing in place for community-dwelling older adults. Her current work extends beyond the walls of the hospital to improve the health of the population living in Yishun Health catchment zones. She collaborates with agencies from health, social and the arts to influence mind-sets and to shape practice on the ground in issues ranging from health and fitness promotion, asset-based community development, strengths-based self-management, active ageing through frailty prevention and advance care planning.

As the Deputy Chairman, Medical Board, in charge of Community Care and Population Health, she assists the CMB in clinical guidance and oversight in the conceptualisation, development and deployment of new clinical care models in the community. She plays a key role as part of the clinical leadership to build a self-sustaining and resilient community network in the population that Yishun Health serves.
