Wang Ling joined Tan Tock Seng Hospital- Virtual Hospital Program in 2015. Subsequently, the team expanded and emerged to Transitional Care Service and now as Community Health Team. She has great passion and is committed in supporting elderly in community setting across different zones in Central Health. She has participated in multiple Community Screening events and Healthier SG roadshow. She also conducts educational talks on Chronic Disease Management, teaches residents/volunteers on blood pressure and blood sugar monitoring via small workshop. Her next move will focus on nutrition and frailty in community dwelling elderly. She has led community nurse post at IDeAL@115 (Integrated Dementia Assisted living) since 2021 to assist community partners and elderly to identify issues, apply clinical interventions and link up resources from hospital/polyclinic/AIC and other sectors. Currently she takes care seniors at seven nurse posts at Ang Mo Kio area and works with various community partners for collaborations. She also won SHBC- Best Poster Award (Nursing) Bronze in 2022.
