Opening Ceremony: Towards Healthier SG (Level 1, Hall 1, Apex)

PANEL SYNOPSIS

At the 2022 Committee of Supply, MOH launched Healthier SG to promote healthier living through a life course approach. Residents are encouraged to build a trusted relationship with a regular family doctor through enrolment and to develop a personalised health plan. Through this relationship, residents will receive support from their chosen family doctor to better manage their health conditions, health concerns and to set health goals.

This panel discussion brings together esteemed panellists from the government, private and public healthcare sectors to share their perspectives of how residents can take charge of their health and participate in this new vision of healthcare. The panellists will also discuss the opportunities and value propositions that could be gained across the government, providers and residents, and the roles that each party can play to make Healthier SG a success.

The recent COVID-19 crisis has challenged the conventional model of managing our workforce. Shortage of staff to manage the surge in cases was a pertinent issue, which was compounded by staff affected by COVID-19 and staff fatigue. TRACK SYNOPSIS

As we make the push for population health and a Healthier SG, we need to learn how to work better together, what shape and form our partnerships should take, and also how to finance this network. In the UK, they are developing Integrated Care Systems where health and social entities come together through contracting arrangements to care for the population.  There have been some success stories where they achieved outcomes like reduction of ED attendances, bed days, SOC visits and financial savings. We hope to achieve the same in Singapore by learning how to work with our primary and community care partners better.

Through this session’s plenary and panel discussion, we hope to create more awareness about Integrated Care Systems and how they could work in Singapore. Our panellists will represent perspectives from the Regional Health System, primary care and community care to discuss what it means to form care networks and develop communities of care to support our residents to live well and age well in the community.

Track 1A: Population Health: What Does it Mean for Providers, Patients and Residents (Level 1, Hall 1, ApeX)

TRACK SYNOPSIS

How can people who live with long term conditions be better prepared to have meaningful strengths-based conversations during their consultations with their doctors, and feel supported between clinic visits to self-manage?

Personalised Care and Support Planning (PCSP) is a systematic process and practice that offers the opportunity of an equal partnership approach between professionals and residents living with long term conditions. It recognises the primacy of residents in their decision making about what matters to them. As they plan their actions to achieve care goals made with them based on their abilities and the community support around them, it gives them the confidence to live well with their long term conditions. Purposeful investment in a system-based support of PCSP has been shown to transform the value and effectiveness of care.

This session will start with a presentation by speakers from Year of Care Partnerships under NHS UK on the principles of the PCSP approach, followed by sharing by local practitioners on preparing and involving residents in their care at the clinic, and the importance of building community based support. We will round up the session with a dynamic panel discussion with representatives from NHG, NUHS and SingHealth to expound on what population health means for us as providers and residents.

Track Synopsis

Healthier SG is a key multi-year national strategy by MOH to address the challenges of an ageing population and improve population health outcomes with a focus on preventive health and early intervention.

Primary care enrolment programme is central to Healthier SG whereby each resident will be invited to enrol with a family physician of their choice. The doctor will then follow up the patient regularly and discuss health goals in a care plan, thereby ensuring continuity of care.

Community partnerships is a key enabler to support better health. The regional health clusters will work with various agencies to provide services and programmes to residents so that they can achieve their health goals.

Track 2B: Beyond Information towards Transformation through enabling Therapeutic Patient Education (TPE) (Level 2, Garnet 212 – 214)

Track Synopsis

Effective patient education goes beyond handing out standard patient information leaflets. Therapeutic Patient Education (TPE) as defined by World Health Organisation (WHO) is education that helps patients acquire or maintain the competencies they need to manage as well as possible their lives with a chronic disease. It is an integral part and continuing part of patient care. It is education that aims to transform the lives of patients and their families living with chronic diseases.

This track will describe the Past, Present and the Future of TPE in NHG with a focus in diabetes care, starting with the Past – what education was prior to NHG DM School; the Present – the NHG DM School Project which started in March 2020 by the NHG DM Education Workgroup under the remit of the NHG Diabetes and Metabolic Steering Committee and moving into the Future – the roadmap ahead.

With increasing pressure on hospital beds, and emergency departments, alternatives to hospital admission are attractive both for patients and healthcare systems.

In the UK, Ambulatory Emergency Care (AEC) has had a huge impact on reducing short stay hospital admissions.

AEC involves streamlining the management of patients that would traditionally be admitted to hospital. They instead can be treated in an ambulatory setting and discharged the same day with follow up, treatment and tests on subsequent days if needed as an outpatient.

In this session, UK based experts will share the nuts and bolts of AEC with some clinical examples. We will hear from KTPH who have some experience setting up AEC in Singapore. Finally, we’ll hear about other admission avoidance strategies and the evidence base for them.

It has been a rollercoaster of uncertainty during the pandemic for everyone and particularly challenging for palliative care field which strives to provide comfort and support for patients and families. In this track, we would like to show case the diverse and collaborative work with various stakeholders on embracing the challenges while we move forward with the endemic.
