The Primary Care Forum is an annual conference for primary care professionals. This year’s theme is titled Primary Care: Innovative Communities, Co-design for Quality.
In this era of increasing non-communicable disease burden and an ageing population, healthcare systems have become aware of the need to go upstream beyond looking after patients at the door of the medical home to how health and wellness can be imbued in the broader community. During the forum, our invited speakers will explore further how healthy living can be promoted through innovative means for the population at large.
Primary care continues to be focused on patient-centeredness to deliver high quality care. Designing healthcare with the patient in mind is a concept that has been gaining traction in Singapore. We hope to hear from several experts the nuts and bolts on how to use consumer experience as a factor in designing a care delivery system in our push for excellent patient-centered primary care.
We are delighted to have Professor Kevin Grumbach who is Hellman Endowed Professor and Chair of the Department of Family and Community Medicine at the University of California, San Francisco.
At our Saturday General Practice Symposium, we will have various specialists speak on a range of topics focusing on metabolic conditions across age groups, from paediatric to geriatric, reflecting the clinical breadth our GPs encounter in day to day practice.
We look forward to you joining us at Primary Care Forum 2019.
