The Intensive Case Management (ICM) Teamlet model emphasises the importance of having a Single Point of Contact, a specific CM to remain as the single health-carer in patient’s journey towards recovery. The ICM strategies include:
A) Increased telephonic support
B) Meeting patients more frequently in outpatient visits
C) Linking up patients with appropriate community resources
D) Follow up patient by dedicated CM during admission and post discharge.
During the Covid-19 Restrictions B), C) & D) interventions were restricted. This paper explores the clinical outcomes before and during Covid restrictions
Datamining of patient’s cases managed from August 2018 to May 2021. Results analysed using Microsoft Excel program.

The ICM/Teamlet model emphasises the importance of having a Single Point of Contact allowing the specific CM to remain the single health-carer in patient’s journey towards recovery. The Covid-19 restrictions may have restricted the comprehensive range of ICM interventions, however more telephonic case management resulted in minimal increases in readmissions and defaults.
