Exploring the relationship between burnout and team development in a primary care setting: A cross-sectional study

Background & Hypothesis

Team-based care was enhanced in NHG Polyclinics through the teamlet care model. Each teamlet consists of two family physicians, one care manager and one care coordinator serving a fixed panel of patients. Studies have shown that team-based care is foundational to improving quality, cost and access of care. However, team member characteristics may affect team development, and presence of burnout may undermine team effectiveness. This study aimed to measure team development and burnout among teamlet members and to investigate the factors affecting team development.

Methods

A cross-sectional study using a self-administered questionnaire was conducted for all teamlet members. Data were collected using the Team Development Measure (TDM) and the Maslach Burnout Inventory Human services survey (MBI-HSS). TDM and MBI-HSS scores were compared amongst professional groups and the relationship between TDM and MBI-HSS was explored using correlation analysis and multivariable linear regression.

Results

A total of 60 family physicians, 28 care managers and 24 care coordinators completed the survey, giving a response rate of 75.7%. Family physicians reported the highest burnout (43.3%) among the three professions. However, there were no significant differences in TDM scores reported by the three professions. Multivariable linear regression revealed that higher personal accomplishment and social interaction outside of work were significantly associated with higher TDM scores.

Discussion & Conclusion

Steps should be taken to address the higher levels of burnout experienced by family physicians. The association between personal accomplishment and social interaction outside work with TDM scores could inform the development of future interventions aimed at improving team development.
