Dr Sabrina Wong is a Senior Consultant Family Physician at the National Healthcare Group Polyclinics (NHGP) and a Deputy Director of Clinical Services. She has led a multi-disciplinary team in implementing teamlet care for continuous and comprehensive care throughout NHGP, and is also leading the Patient Activation through Coaching and support (PACT) program. Her areas of interest include developing innovative chronic care models to support chronic disease outcomes, especially in providing support for self-care management and psychological health.
